How to Request an Itemized Medical Bill
Before you pay a medical bill, ask for the itemized version. The summary you usually receive shows a total, not the line items, and you cannot catch an error you cannot see. Studies have found that a large share of itemized medical bills contain at least one mistake.
Why the itemized bill matters
An itemized bill lists every charge with its billing code, quantity, and price. That detail is where the errors hide: duplicate charges, services you never received, wrong codes, and quantity mistakes. None of that is visible on a one-line summary.
Requesting it is your first and best move to lower a medical bill, before you negotiate or pay anything.
How to request it
Contact the billing department and ask for a fully itemized bill showing every line item, the CPT or billing codes, quantities, and unit prices. Ask them to send it as a PDF or by mail, and request it within a reasonable window such as 15 days.
While you are at it, ask for a copy of their financial assistance or charity care application, in case you qualify for help with the balance.
What to do with it
Check each line against your Explanation of Benefits from your insurer. Flag anything duplicated, unfamiliar, or miscoded, and ask the provider to correct it. Then, if a balance remains, you are in a strong position to negotiate or request charity care.
Catching even one error can save real money, and it costs nothing but a phone call and a careful read.

Why request an itemized bill?
Because errors hide in the line items. A summary bill shows only a total. The itemized version, with billing codes, lets you catch duplicate charges and mistakes before you pay.
Can the provider refuse?
You are entitled to an itemized statement of your charges. Ask the billing department in writing and keep a copy of your request.
What do I do after I get it?
Check each line against your Explanation of Benefits, dispute any errors, and then negotiate the remaining balance or apply for charity care if you qualify.
